League bowling sheet/summary

Im on a duckpin league and each week our league manager gives out the little 4" x 8" score/stat sheets that we write down our score for each game and the number of spares/strikes each game and such. I cant find anywhere online a copy or template of this or even for sale anywhere. Does anyone know where I can buy these or find a template? I dont even know exactly what they're called either, so any help would be appreciated.

Hey Rod, I just started keeping track of that stuff in our league 3 years ago. With no auto-matic scoring not easy. I count up the spares and strikes etc. I made up our own weekly score sheets and added in the extra. Made it on a regular 8.5x11 sheet of card stock paper. Found out, with the bigger sheet, that the numbers are bigger, and makes for less mistakes
